Requirements
NEW APPLICATION
- a) A simply signed Power of Attorney;
- b) Applicant's name, address and nationality;
- c) List/s of goods and/or services;
- d) Representation of the Mark, preferably in JPEG format; and,
- e) If priority is claimed, a certified copy of application/registration
RENEWAL
- a) A simply signed Power of Attorney and,
- b) A copy of the existing Certificate of Registration
ASSIGNMENT
- a) A simply signed Power of Attorney;
- b) A copy of the Existing Certificate of Registration and;
- c) A notarized Deed of Assignment
AMENDMENT
- a) A simply signed Power of Attorney,
- b) A copy of the exisiting Certificate of Registration and,
- c) A notarized document proving the ammendment
